In the box below, enter the first four letters of your legal family name and the first three letters of your legal first name followed by your 11-digit AADSAS application ID, beginning with "2015". For example, Joseph Smith would enter "SMIT JOS 20151234567". Enter this number carefully and accurately as this is the only way our office will be able to match your payment to your application; otherwise, your supplemental application fee will not be processed and your file will not be advanced in our application process. After submitting this information, you will be taken to our payment website.
It is important that you retain your confirmation receipt and order number that you receive from QuikPAY. If you do not receive an AADSAS confirmation within 1 week after submitting your application, notify the office of admissions at firstname.lastname@example.org with your confirmation receipt. If you receive an AADSAS application fee waiver, forward this letter to the office of admissions along with your confirmation receipt. Note that your supplemental fee is NOT refundable unless you are granted a Fee Assistance Program waiver from ADEA.
