Patient Information Sheets Policies and Procedures

PURPOSE

To inform patients of the risks and benefits of various treatments provided in the DMD clinics.


POLICY:

The applicable Patient Information Sheet(s) must be given to patients during treatment plan presentations, and at such times when appropriate to clarify the patient’s understanding of treatment to be rendered in the SDM.


PROCEDURES:

When the treatment plan(s) are explained to patients, students must give the applicable Patient information Sheet(s) to patient and enter the ID of the Patient Information Sheet in the treatment plan. The content of all discussions with the patient regarding benefits and risks must also be summarized in the progress notes. Failure to document that patient information sheets were presented to the patient may result in a student being assigned additional record audits.
